MHSAA Communicable Disease Form
The link above is to download the official Communicable Disease Form from the MHSAA. This form is needed for any wrestler with a questionable skin condition to pass skin checks at any match or tournament. The form needs to be filled out completely and signed by the doctor. The skin condition must be considered "Non Communicable" in order for the wrestler to be able to compete. Parents are advised to make a few copies of the completed form and provide at least one to the coaching staff.
Any questionable skin condition must first be brought to the coaching staff's attention, and then be thoroughly examined by a doctor. This is to insure the other wrestlers are staying clean and not infected. |
