Ohio High School Athletic Association Physical Evalutation form.  This must be completed prior to a student athletes' participation in the Riverside wrestling program

 

 

 

 

Parental permision for the release and publication of a student's photographic image.  Please sign this yearly so Riverside may release your student/athletes' image to media outlets interested in Riverside wrestling. Such as newpapers, magazines, internet and for use on our website.
